The VistA Hospital Information System uses an active Data Dictionary to define all fields. Data Entry and Data Output programs in the Database Manager named "FileMan" all depend on the definitions of the fields, which are stored in the Data Dictionary, and act as tables that define all the data.
The Meta information is limited, but the active use of the Dictionary allows consistency across all use of the entries (rows) and fields (columns) in the database. The VistA system has been in active use for over 30 years to provide an enterprise wide information system supporting health care for veterans in the US.
